Fees

The fees for the examination by the physician, review and consolidation of medical records, and determination of eligibility for Medical Marijuana are:

New Patients………………………………………………$195.00

Renewing Patients………………………………………,,,,$150.00 ** This is for renewing patients that have been seen at our clinic previously.

The State will only accept medical recommendations within 90 days of a physicians signature on the recommendation/statement. Your responsibility is to ensure your office visit is within 90 days of your card.s expiration date and you submit your application within the 90 days to the state.

This will prevent another office visit to our clinic, as the State will deny your application if the medical recommendation is greater than 90 days from the physicians signature. If you do not submit your application to the state and the 90 days from the physicians signature has lapsed, you must schedule another physicians visit nnd pay the appropriate fee.
